Injury risk during the football season affects players at all levels

At PRP Health, we commonly see junior athletes presenting with ongoing calf and heel pain, particularly during the football season. In many cases, this discomfort is related to Sever’s disease, a common growth related condition that affects active children and adolescents.
Sever’s disease, also known as calcaneal apophysitis, occurs when the Achilles tendon places increased load on the heel growth plate. During periods of growth, this area of bone is softer and more sensitive to repeated stress. Symptoms often fluctuate and can settle before returning again, particularly during growth spurts. While it is most commonly seen in children aged 8 to 12, symptoms can persist into the teenage years depending on growth patterns and activity demands.
Footwear and training load play an important role in symptom development. Junior athletes often transition rapidly between school shoes, running shoes and football boots as the season begins. Many runners and school shoes provide heel elevation and cushioning that reduce strain through the Achilles tendon. In contrast, football boots are typically flatter and provide minimal midsole support. This sudden change can increase load on the heel and Achilles before the body has time to adapt.
Another common pattern we see is delayed presentation. Many junior athletes continue training and playing through discomfort for weeks before seeking help, often only raising the issue once pain begins to limit performance or participation. By this stage, symptoms may have become more persistent and require a longer period of management.
Early assessment is important. At PRP Health, junior heel and calf pain is assessed by our physiotherapy team, including Adam Russell and Shane Weigman, who work closely with junior athletes and families. Management focuses on accurate diagnosis, load modification, strength and flexibility work, footwear considerations, and a structured return to sport plan.
Addressing symptoms early helps reduce time away from sport and supports safer long-term development for junior athletes.
